We are looking for a dedicated Care Extender for our Tailored Care Management program in Wake County, NC.
The Care Extender provides community integration services, information and referrals, and general advocacy for individuals with intellectual/developmental disabilities to help them increase self-sufficiency and gain access to needed services (including medical, social, educational, and other services).
Job Duties Include:
- Conducting general outreach, engagement, and follow-up with recipients of services.
- Assisting the care team with goal implementation.
- Supporting the Care Manager in assessing and addressing health-related resource needs.
- Sharing information with the care manager and other members of the care team, as appropriate. Notifying appropriate officials of health, safety, and welfare concerns regarding the individual as required.
- Assisting individuals in locating and accessing community supports and resources that are related to achieving their goals, including social and educational resources as well as natural supports.
- Referring and linking individuals to physical and behavioral health services in the community to ensure necessary support systems are in place.
- Collaborating with other service providers and agencies.
- Documenting program activities (including progress notes and service trackers) as required.
- Coordinating 24/7 crisis call response as needed. Participating in the organization’s on-call structure.
- Advocating for and promoting the full inclusion of individuals with disabilities in all opportunities in the community.
MINIMUM REQUIREMENTS:
Education: A Bachelor’s degree in Social Work, Human Services, Psychology, or related field is required.
Experience: Minimum of 2 years accumulated experience with the population served, pre- or post-Bachelor’s degree required.
License/Certification: Current CPR and First Aid certification required.
A valid driver’s license and auto insurance is required to meet the requirements of the position.
