RESPONSIBILITIES INCLUDE:
Ongoing outreach
Attempts ongoing outreach and engagement with unengaged consumers
- Supports the Care Manager in assessing and addressing SDOH needs.
- Assists with the intake by meeting with the family and getting consents signed
- Shares information with the assigned Care Manager about their case
Providing Support
- Assists individuals in locating and accessing community supports and resources that are related to achieving the associated goal
- Keeps the Care Manager updated on progress and any newly identified needs
- Supports the Care Manager in assessing and addressing health-related resource needs
- Notifies appropriate officials of health, safety, and welfare concerns regarding the individual as required.
- Collaborates with other providers of service.
- Refers and links consumers to non-Medicaid services in the community
- Follows up with families to assess progress and ongoing needs
Complete Documentation
- Obtains and uploads all supporting documentation, Legally Responsible Person (LRP) verification, and release of information to appropriate EHR
- Documents all activities performed with and on behalf of the consumer in the appropriate EHR and tracker/spreadsheet
- Ensures that service orders/doctor’s orders are obtained, as applicable
- Shares appropriate documentation with all members of the care team
- Ensures all clinical documentation (e.g., goals, plans, progress notes, etc.) meets state, agency documentation standards, and Medicaid requirements
24:7 Call Crisis Response Coordination
- Identifies and provides emergency crisis response as necessary, following agency policies related to crisis.
- Participates in post crisis team debriefing and provide feedback on ways to prevent future crises for the consumer.
- Participation in agency on-call structure is required.
Minimum Requirements:
Education: A high school diploma or GED required. Bachelor’s degree in Social Work, Human Services, Psychology, or related field required.
Experience: For QP status, minimum of two (2) years, pre- or post bachelor’s degree accumulated experience with the population served, or one (1) year pre- or post-graduate degree accumulated experience with the population served.
License/Certification: Current CPR and First Aid certification required.
Knowledge: Knowledge of specific aspects of service provision and population served that would be necessary to perform the technical functions of the position (including crisis management and on-call responsibilities). Knowledge of the principles of self-determination, freedom, and civil rights as applied to a traditional case management model of service delivery.
Other: Demonstrated proficiency with MS Office software. Transportation and the ability to transport consumers is required to meet the requirements of this position. Ability to provide 24-hour on-call service is also required.
